Provides comprehensive, community based care to persons who:

• elders who are 55 years of age or older,
• residents who live in the desinated service area for the ESP program in which they are enrolling,
• Certification by the MassHealth's Office of long term care screening agent that the elder meets the Medicaid nursing facility clinical criteria.
• Elders able to stay in thier homes safely ESP support services.
