Narrative

State Home Care: Provides home care & supportive services to eligible elders, 60+ years on donation or sliding fee basis. (Town served are Belchertown, Chicopee, Granby, Holyoke, Ludlow, South Hadley, Ware)  Separate listings. MassHealth Programs for Disabled in Western Mass, including under 60.

We do not provide medical services, and do not bill insurance providers.  We do provide some MassHealth Programs that are authorized through MassHealth, such as Adult Foster Care and Personal Care Assistance Program.  Please contact us for more details.
