Narrative
The Wise Adult Day Health Program is a center for adults with health related issues. Their ability to function independently is limited. They may need supervision or help during the day when caregivers can’t be with them.
Program Goals:
- To provide a safe, supportive setting which allows people to maximize their quality of life and help maintain their independence.
- To enhance the person’s health and safety. We provide medical monitoring and preventive health care to participants
- To provide group and individualized, self-directed social and leisure experiences for people to promote friendships and socialization.
