Narrative

Easter Seals Youth Transition Services

Providing transition services to assist youth with disabilities through:

  • Comprehensive Transition Assessments  and
  • Individualized Transition Coaching

 

Coordinated Assessments (roadmap):

 

Transition

  • Age-appropriate, student-centered
  • Transition Planning, including Post-Secondary Vision/Goals in Education/Training, Employment and Adult Living
  • Transition focused IEP goals
  • Provides a guideline for students, parents and educators to use when planning for appropriate and realistic employment goals

Assistive Technology/Augmentative and Alternative Communication/Curriculum Access

    • Augmentative and Alternative Communication (AAC) Evaluations for students with expressive communication and comprehension needs
    • Assistive Technology (AT) Evaluations for curriculum access for students with technology access and educational technology needs to plan for independent living, higher education and work

Vocational

    • Appropriate vocational development activities and transition service recommendations are identified for the student
    • Provides a guideline for students, parents and educators to use when planning for appropriate and realistic employment goals

Rehabilitation Therapy/Evaluations

    • Occupational Therapy/ Physical Therapy/ Speech Language Pathology

Transition Coaching supports (journey):

  • Personalized real-world learning experiences in employment and independent living
  • Building self-determination and advocacy skills through experience and exposure to new challenges
  • Assisting families and youth with navigating and coordinating the complex maze of adult services

Easter Seals Transition Services are designed to work with youth and families to develop a road map, and provide the guidance necessary for a successful journey.
